Weight management

Use this service to check your eligibility for a referral to an NHS weight management service.

The weight management services support adults living with obesity who also have a diagnosis of diabetes, high blood pressure (hypertension) or both, to manage their weight and improve their health.

You can find information and advice for adults and children about healthy weight on the NHS website.

• are registered at the surgery
